Why an Unlinked Health Record Can Hide the Real Problem

Logging a health event is only half the job. The other half, connecting it to what came before and what happens next, is often where the actually useful insight lives, and it's the part that's easiest to lose when events are recorded as isolated entries rather than a connected chain.

Missed follow-up appointments are a recognised, preventable driver of post-treatment complications.
